How can these flakes be applied to paper?

Try applying Extra Thick Metal Leaf Adhesive or Spray Gold Leaf Adhesive in the areas where gold flakes are desired. Use a paint brush to remove excess flakes.

What is better to use during the first clear top coat (while it is still tacky), metal flakes or metallic mica flakes?

The mica powders are very fine powders, so the metal flakes would be a better product to use. However, the sizes of the flakes will vary in each three-gram bag.
